It isn’t because of chain stores, but straight up Sony, Microsoft and Nintendo. Remember Manhunt 2? It had to be reworked because Sony and Nintendo refused to let the game out due to an AO rating.

You’ll notice that the only AO rated games are for PC, since there’s no manufacturer behind it (well.. there’s one for CD-i).
