First of all, of course you can still scream with whatever gag, because, you know, there is NO gag that can completely muffle all sounds. You’d have to incapacitate the vocal cords for that (however you’d do that. I don’t even want to imagine).
Secondly, Asian breathing/drooling-type ball-gags are terribly ineffective.
