Wait, so is Tissue Hime a guy or a girl?... you said "he" in your post, and people have been saying "guy" in the comments, and the cake in the end features a combination of ♂ and ♀...
He's never said what he is. But I think Hime's a he mostly because of that cosplay book he holds up at the end of the Triangular video. It reads "I want to be a girl", and I'm just taking it as a hint. Hime has a blog too. Can't seem to find it atm.
But then again... Hime's got unusually fine legs for a guy.

Awesome performance, I can actually picture Nagato siting there playing.^^
But to answer the girl or guy question... GUY, hands down. His legs look so fine because hes wearing a kigurumi costume. Not everyone can rock those, but it seems to suit him very well. He could wear the matching char mask except it may ruin the illusion. Example Below and some cool related images:

I don't really know... I thought the legs look surprisingly like a girls because of the size/shape. Hmmmmm... and pertaining to the kigurumi comment, I don't really think Hime is wearing the whole body costume. Not only will it look slightly bulky, wearing the costume will make the skin tone look unnatural and fabric-like. From the video, you can see that Hime's legs look pretty realistic. And not fabric-like either. You can tell by looking at the reflection of the white sheets on Hime's legs.

The shape of the legs do look a girls, but I sort of wrote that off because I thought: "why world girl need to wear a full body costume?" Probably to add to the mystique? I only assumed it was kigurumi costume, because it seemed like it was all one piece and leg skinned looked sort of fabric-like. But when I looked at the legs and the bed I saw that you were right. I might have been confused by the lighting.So it could quite possibly be a girl just trying to MINDFUCK us into thinking shes a guy. But i guess we'll never know...
