I think youre forgetting how many times Rika from the Higurashi series died so that all her friends wouldnt die, wouldnt kill each other, wouldnt succumb to a disease, wasn't tested on, etc
By the math I've done, it's anywhere between 100 years or 3.7 million deaths. I can't get a clear number because the content I could get the do this was vague, fan translated or moonrune (which I'm not good at)

n.b. I realise that Index being a nun is all in good fun and not in any way actually like being a nun. And personally, I'm enthralled by one of the show's other nuns, Orsola, who is a wondrous delight of a woman, and shouldn't be a nun.

From the list above, I'd support Homura the most. Someone who should on the list (and high on it) is Nagato. Her plight was similar to Homura, but maybe even worse.
She was assigned the job of monitoring a mildly crazy sociopathic god and having to put up with many of the indignities that went along with that (although nowhere near as many as the lovely Asahina-san had to). She falls in love with her perfect human but can't have him even though he likes her. She changes the universe in order to give him another chance and again gets nowhere. So she has to live out her days, such as they are, watching and eating her liver.
She's smart, cute, capable, bold, gets things done and is doomed to get absolutely nothing.

The hell is Index doing here? Who would pity her? Her backstory is cruel enough, but afterwards she dedicated her life to making Touma's life as miserable as possible despite the fact he was a total outsider who still risked his life numerous times to save her. Index should be in the Top 10 most ungrateful anime characters list, not here.
Because he won't stick his dick in her as he is too busy fooling around with Biribiri, Kanzaki, Oriana, Orsola, Seiri etc. etc. etc.

She has one of, if not THE most, tragic lives of all the Fate girls. Getting evil magic sex worms implanted into her at a very young age. Being constantly physically/verbally/sexually abused by her adopted brother (Hell, just having to live with Shinji is bad enough). Usually being romantically ignored by Senpai (having 1 out of 3 routes in the original game, and she has to share hers with the effectively fourth heroine that is Illya).
After her betrayal of Kariya in Zero, my tears dried up.
