Actually, there've been quite a few console games with full nudity in Japan. Bootleg X-rated games can be found for the Famicom (NES) as well as the Super Famicom (SNES) and Mega Drive (Genesis).
Licensed X-rated games can be found on: Sega Saturn, 3DO, PC-FX, and Neo Geo CD. One official X-rated game exists for the Neo Geo Pocket (Super Real Mahjong), and there's been a ton unofficial releases for the PSP which are ports of Japanese adult computer games, with full nudity, sex, what have you.
