The weapons had thorns on them so you could assume that once the weapon pierced them they would be unable to remove it, and as long as it stayed in it would do continuous damage(when the guy got impaled where the sword was in his body glowed purple occasionally so that might indicate him taking damage). Also the game does more damage based on where one is hit, so if it hit her heart it would most likely do extra damage along with the damage over time.

You're wrong about higurashi, they actually die in the beginning of the first episodes' "flashforward" and then at the end of episode four(or five) when it actually takes place...
