There is one thing though I've been getting increasingly confused with: is Misaka or Mikoto her given name? It should be Mikoto, but everyone's using Misaka as if that was her first name... probably because that's what the clones are referring to themselves as?
Her given name is Mikoto, but I don't think many people call her that in the series. They usually use her family name. That's probably why most people call her Misaka when discussing her character/the series/etc.
Yeah, it's pretty common for people to just call each other by their last names in Japan. Hell, some think that addressing other people by their first name, even with honorifics, is indicative that they're in an intimate relationship.
On a side note, this Last Order is preying on my weakness for moe, and the gravitational pull is freakishly strong. >.>;
