In my experience with that show (and manga) there isn't really a lot for fujoshi fantasy asides of fours supposedly handsome guys (no, sometimes only that is not enough to label something as BL inducing). That show revolves more around Sunako and comedic stuff, not that much BL.

maybe it's because the Hetalia anime is pretty much web-based. all the other anime up there are broadcasted on national tv, while all the fangirls generally lurk around the author's blog and talk about X in their off time. this could have figured in the rankings.
