I've lived in Japan. The otaku girls I saw in the anime stores and browsing the doujinshi shelves looked like any other fashionable girl on the street. It's impossible to know by looking if a girl on the street is an otaku or not. And most of the amazing sexy cosplayers I see are huge otaku.
