Saimoe is a surprisingly complex thing for what it entails. It is not just "characters duking it out." It is series duking it out, series backing other characters in order to take out competitors, revenge votes from series against other characters (who took out their beloveds), fake vote spams that mess up the preliminary tallies to try to manipulate voting by creating a false sense of security or a false sense of need, attempts to organize your faction's votes at a late time so that the opposing faction will not be able to drum up more votes.
It is never just a popularity poll for two to four characters.
Mio was taken out during a period where she was most vulnerable, the first round. Fans of other series can back someone they don't care for but who has the best chance of beating a target (in this case Kana from Minami-ke) as well as take advantage of the disorganization where factions are not yet fully in tune. Mio had 526 votes, Kana 563. K-on fans made sure to properly back Yui who had 708, clearly above anyone in her block for the round and ensuring that there would be no surprises.
Haruhi was a victim of its anime bullshit causing displeasure in its fanbase. In 2006 they had much more votes than they did this year. Both Haruhi and Yuki later faced off against Eruru from Utawarerumono and their combined votes were barely above that of their competition.
