A man has been strangled to death by a robot arm.
The man, a 46-year-old employee of a Tochigi prefecture PET bottle manufacturing plant, was working a graveyard shift with two others supervising the operation of plant machinery, in this case a robot aligning bottles on a conveyor belt (possibly this simply describes righting any bottles which fall over).
Somehow he managed to interpose himself between the arm and the machine to which it was attached, and the machine caught him by the neck, crushing it and mortally wounding him.
He arrived at hospital 30 minutes later, but was confirmed dead.
Police are investigating the cause of the incident, though presumably the robot is not a suspect at this time.

This kind of fatal accidents usually occur when a person operating the process does not follow the safety procedures or the safety of said process is flawed.
Also sometimes the operator is not informed enough about the hazards operating the process, so there's on more possible reason for this accident.
Still, I would go for the ignoring of safety procedures as the cause of this one.

The trouble with robots is people. People who specify them, people who program them, people who buy them, and people who work with them.
Did the robot's designers omit reasonable safety features on a robot designed to work in proximity to humans?
Did a robot manufacturer's manager decide that the safety features were "too expensive", and order them deleted from the design?
Did a robot company programmer have an "oops" moment? (Humans are not infallible.)
Did a robot company manager decide "there's no time to do all the program code testing/quality assurance -- just ship it"?
Did the bottling company fail to check whether the robot was designed to work with humans in the vicinity, or, simply buy the "not designed for humans in the vicinity" model, because it was cheaper or could be delivered more quickly?
Was the worker a dumb-ass who carelessly ignored the safety regs and common sense?
Was the worker pressured by management to ignore the safety regs and common sense?
Did the worker just have a brain fart, made one mistake, and one thing lead to another?
I've seen industrial robots with human-safe features demonstrated. The guy put his hand in the way of the arm's path, and when it hit his hand, the robot froze in-place. I was told it was programmed to halt if the arm encountered more than 12 pounds resistance.
I was impressed at the time. I didn't think then to ask whether that safety feature also applied to the gripping claw. Twelve pounds of pressure is, I'll guess, enough to crush a human throat.
Well consindering Japanese invented some life like Androids.
I for one welcome our new robot overlords.

In all seriousness though, it sounds to me like the guy just put his neck where it shouldn't have been and he got killed. Machinery has been killing people this way since the 1800's when the 10-year-old immigrant children working in meat packing factories would get crushed while operating a conveyor belt or something. It's not really the rise of the machines.

No, I'm just being reasonable. The enemy A.I. in your average shooter probably has a better chance of becoming self aware than the A.I. of a robot arm designed to pick up bottles that fall over on a conveyor-belt. It's something like an A.I. powerful enough to sort and process a huge amount of data that you have to watch out for.
