Definitely Photoshop! I use it for a living and its not even a very good example, he has no shadow where the girl does, his arms cropped a bit funnily; also he's at a different angle to the toddler and the camera focus is above her completely.
It is a shop but, this mindset is why that poor Italian man in Brazil got put in jail.
