It is indeed the real manga. At first I was suprised too (I mean, there's no real sex in KissxSis, as far as I remember), until I realized that the stick she is licking is, in fact, a popsicle. xD
Actually one of the latest chapters I would consider containing real sex (two of them screwing through his boxers), and if high school health classes aren't filling us with bullshit it's possible one of the girls got pregnant from it but just doesn't know it yet.
