That's because of lag :( Most of the time headshots wouldn't kill, or despite hitting in melee I wouldn't get them off me, or... even worse, after a sure kill hit, I'd read "assist" points, without ever touching them before.
Concerning the last two games as a vanguard, I just suck.

you need to practice more. play more games and stuff. that's the only way to get better :P
dunno, in my opinion the vanguard just sucks. Best character out there is the krogan sentinel, so far!
