Personally, I think the final answer to this problem lies in our ancestral diet. Hunter-gatherers are never obese, despite humans not having much self-control by our nature. They never give the appearance of starving either, unless their territory is being intruded upon by modern society. And it's not even like they exercise a ton. They walk around a lot, but most of their heavy exercise comes from a (maybe once a week?) hunt, hauling the carcass back to camp, and playing games to pass the time. Despite that, the majority of them have bodies like this:

True. I agree that diet makes a bigger difference compared to how much exercise you get (as long as you don't sit all day).

Diet makes a bigger difference to "leanness", that's for darn sure. Although extreme levels of exercise and muscle mass can produce fat loss with a very high calorie diet.
But muscle size can be achieved with a pretty sloppy diet as long as protein and calories are high and there's the right kind of exercise.
------
The thing about hunter gatherers is that they tend to have lower carbohydrate and higher protein intakes. Their sugar intake is especially low, and sugar is horrendous when it comes to fat gain.
Sodium intake is low as well, which means they don't look bloated due to water retention.

Although once you gain major muscles, won't you have to maintain it by keeping up the same extreme level of exercise all the time, lest that all turn into saggy stuff?

No.
Muscles only turn "saggy" when you stop the bodybuilder exercise routine but keep the bodybuilder diet.
And that's because they're actually just disappearing while fat levels are increasing.
God-tier advice:
Took 10 days off of weight training, just for recovery purposes. After several months I had to let my tendons rest, had little issues with my shoulder and groin, sore neck, knee, and back.
I was still supplementing and eating like an animal just to take advantage of my inactivity to gain weight.
I began hitting the iron a few days ago and my body felt great, I picked up right where I left off.
I need to start working out again. I'm not fat yet but when I slap my stomach there is actually a loud clapping sound now so I think i'm on the verge of becoming fat.
I've been working out everyday.
I needs a lucha libre body.
I either eat if I don't care about weight and weigh 120 to 125 or seriously restrict and am around 110-115. Workout has little to do with weight if you only do it moderately to occasionally. It may tighten you up a smidgen but from what I've learned it's mainly about how much you eat. I go through periods wear I am seriously ripped and periods that I'm little chubby bunny. But I'm a mesomorph so it's easy for me to lose weight and pack on muscle.

I've heard it said that (as far as staying slim is concerned), it's 80% diet and 20% exercise. If one thinks purely in terms of weight, that's true.
But I'd say that it's pretty easy to tell when someone is slim because they work out or just because they don't eat much. Slim people who don't work out are often skinnyfats who look surprisingly bad when naked and actually hold quite a bit of blubber. Unfortunately, most female porn stars fall into this category.

There is nothing worse than a skinny redneck with a muffin-top and a beer-gut hanging two inches down over his belt.

Don't ever go on holiday in Spain or Greece. You'll want to kill yourself when you see all the skinnyfat Brits. They all look like this:

Oh yes, I do know quite a lot of skinny fats. I think they are just ectomorphs that do not work out. They stay skinny because of their metabolism, but they do not work out at all. Their body naturally has lean muscle but if they don't work out that will be hidden with a layer of flappy blubber.

I see this too much right now in the summertime being from rural NC.

I actually prefer legitimately fat dudes (as long as they're not morbidly obese) to skinnyfats. At least fat dudes look pretty strong.

That is true. Personally I judge the fat itself. Sometimes it is close knit and looks sturdy, while other fat looks flappy and gross. I don't know if that makes sense. But to my eyes it makes a big difference.

No, that makes a lot of sense, and is almost the exact same thing that I said to a friend of mine the other day. Especially for upper arms. If a person holds his arm up, and it stays (fairly) in a cylinder, then ok. But if it sags down like a sail in the breeze, then there's a problem.
