In Japan usually the girl still have a petite-ish torso but with big preasts on it so even if she had a flat chest she would stll look girly. In America the comics big breasted girl often have a larger torso too in a way that the breasts aren't wider than the torso so if you remove the tits she would end looking like a guy.
