I personally like Mikoto as well, but she’s not necessarily flat chested either. She’s in middle school still, both her older clone and mother have large breasts, and the official art has been showing her as average size for her age for a while now. She just has a complex about it mostly because people tease her and all the big breasted adults Touma ends up hanging around in his adventures.

Index on the other hand is implied to be well in her upper teens, and her childlike behavior is thanks to having her memories erased multiple times. She’s pretty much the only true pettanko of the series.
