sometimes you can just tell for many different reasons, like the art, title, summery, categories, or the first chapter/episode,
i anyways just make opinions on the first few pages of the manga i read, and if something starts out with jokes, i wont think its going to be that “serious”, if it starts out with someone being decapitated, than i think its going to be a whole lot darker
but when i’m reading something with “cute-like” art, about a magical middle school girl fighting evil, with a cute little follower, my mind doesn’t jump to “it’s going to be a blood-bath”
comparing that too berserk, where all the art looks dark, the title isn’t exactly pink and fluffy, and it starts off with a guy having sex with a girl, who then turns into a monster -followed by him killing it i not really going to be surprised if later i see people getting raped and murdered

I think Elfen Lied should be a little higher on this list, considering the child-rape back story… doesn’t get much more shocking or depressing.
