  Since you brought up the Pico anime, I think you forgot something: In “Boku no Pico” (the first OVA), the old man introduces Pico as his grandSON (according to the fansub). If that’s what the original writer meant, Mokkun’s reaction to Pico’s true sex wouldn’t make sense.
