Than the one making this cleary had no idea what he was doing. As far as those games go any line that is slightly different counts. If there are more lines in a pattern, than the pattern counts.
Also the change of expression counts as one. All the tiny differences you circled on her count as one. So there are only 5.
No, they don't count as one. Not if you play by the rules. One can't just change the whole character and say it's only one difference.

K fags who are over-analyzing, first of all it's fairly obvious that Biribiri having goggles on her head and different eyes, is supposed to be one difference, Biribiri has been replaced with MISAKA.
Second, Kuroko's pose has changed, that in itself is an entire difference, you don't count her hair having moved a little etc. Her expression and location are entirely different, SHE has moved, that is ONE difference.

Uh... huh, most of you missed the (rather obvious) point. It's easy because the differences are supposed to be hilariously goofy. In fact it says "There are *about* 5 differences," just to point out it's carefree and meant to lampoon such childrens' games. Laugh at the straw up a nose, not pat yourselves on the back over a non-existent challenge.
