Maria-sama ga Miteru DVD Disasters Shock Fans

Catholic yuri action anime Maria-sama ga Miteru has attracted much attention from fans for a series of disasters afflicting its DVD releases of late. Most amusingly, above you see the impressive start made with the first DVD release of the 4th season.
Possibly her speechlessness was lent an excessively literal interpretation?
More disturbingly for fans, the recent DVD box set of the first season was also bungled in a manner few fans will be happy about.
One of the major special features the box set was marketed as having, a series of illustration collections entitled “A Secret to Maria-sama”, was actually omitted from 5 of the 7 discs, in a rather impressive display of quality control.
The box set is fortunately not so very expensive, but even so the lack of any response save an apology from the publisher is sure to rankle.

Somehow this reminds me of Minami-kes first season (or was it second?). At some episode (I don't remember which) someone grabbed a piece of other persons shoulder. Minor hilarity ensued.
