It’s also, or at least was, very cery common for racers to be decapitated by a stray wheel in F1.

I think it’s less common now though, because they channged the way they fix them, to prevent such accidents.

Number one: the bolts broke on their own because of defects in the bolts…. and an inspection would not have helped.
