It's 99% safe and despite the look of it there won't be any internal hemorrhages. One of the fascinating qualities of kinbaku/shibari is that it can be extremely tight while still not hindering blood flow at all. That is, if done right - some of the images here show techniques that need a lot of experience to be executed safely. But as far as I can tell, they are.

And of course, some positions are MEANT to be painful...
