Likely, most of these pics are from Russia. Very typical, I can see something like this virtually everyday.
All this are mostly results of reconstruction that is either under progress or stopped due to the lack of money
Buildings with trees in them do exist. It is often illegal to cut down the trees so they build their house around it instead.
In Russia, there are no handicaps! Only handichallenged who are strengthened by hearty structures that make them Big Strong!
I would say that most of these come from eastern europe,,where much of the buildings have been destroyed by war and rebuild using whatever they can find.

The first pic reminds me of a friend who was doing architecture. She asked how close urinals are normally; she had to squeeze a certain amount of them within a room. I told her they would be waaay too close together, but she said "I have to fit them all in or I'll lose marks". I guess the person doing that fitout was in a similar situation...

actually, I'm quite sure most of them are due to the lack of planning and during/before construction. Also, the parts where they have stairs and no entry way is probably just due to laziness.
At least a few of those stairs with no entryways look like the entry was removed at some point, and new wall put in place. The difference in colour (often lighter) is a tell tale there, and it's not uncommon for doors to be removed, particularly for security reasons (ie, in a shop, after enough people dive out a particular exit while shoplifting, someone proposes removing the problem exit entirely).
Some of them look like remodels.. but some of them, I literally have no idea what was going on there. The toilet at the top of the stairs.. O.o;;;? Seriously, WTF?

I actually rather liked the idea of peeing directly into urinal plants. I think that this fulfills a natural inclination. Kind of like blowing loads 2D depictions of flatties.

At around 1:34 you can clearly see a door has been filled in since the initial construction. Also I believe I've seen that bridge one around 4:44 on TV before.
